- Whether you are a new or existing family, please make sure you fill out our brand new Holy Disciples Faith Formation Registration Form and drop off the hard copy at the Faith Formation Office into the white mailbox.

- New Families:  Please register with the parish by filling out a Parish Census Form and submitting it with your Fatih Formation registration.

- If you are registering a new child/student for the program year 2023-2024, please make sure to also fill out New Student Form that is available online and drop off the hard copy at the Faith Formation Office into
the white mailbox together with the Registration Form.  

- Please make sure to make the payment. You can find the payment link on the Registration Form available on the website. Just click the link and fill out the necessary information. 

- Please be advised that we have Registration Form and payment submission due dates and registration closingdate. Please return completed Registration Form, and make payment online by Monday, August 7th, 2023.

- Respectfully, late registrations will be charged $15

- Registration CLOSES Monday, August 21st.
